Please complete the following document (except for the box labelled "For Office Use Only") and bring it to your next appointment with Dr. Goldberg. It is important that we have the information contained in the form on file to comply with Electronic Health Record rules and regulations. Please be as detailed as possible when listing your prescription information. Filling this out prior to your appointment will allow you to see Dr. Goldberg more quickly.
